The game notifies you when they're eligible for parole but they aren't coded to use parole rooms.
They complain about the luxury need but have no way of gaining money to use the shop.
If they get agitated once, you can find yourself in an endless loop because many of them will refuse to pick up a tray and eat in the canteen. The game gives you no way to deal with this directly so they just become more agitated due to hunger or in some cases starve to death.
And as others have said, minus a few little quirks like running in circles, streaking, collecting/stacking junk, and needing to see a psychiatrist, they don't offer much of a gameplay difference from other prisoners.
There are a lot of simple things they could do to give them more depth but considering they haven't bothered to do bug fixes after this long, there's no chance they're going to do anything about it unless they can stuff it in another DLC and charge us for it again.

Either don't buy this DLC or toggle off criminally insane prisoners right away if you don't wont to build specifically a psych ward. This just doesn't mix well with the main game at all.

Note I only recently started playing this game again
This DLC is bad. Why is it bad? Lets delve into that as quickly and short as we can.
1; Criminally insane inmates are just inmates with diplomatic immunity.
Your guards do nothing to them, they're allowed to run about, and ignore all your ruling, and generally the only way to stop them is using the new Orderlies who only work on them. This is silly.
2; Psychiatrists only work on Criminally insane inmates.
Did you think maybe the guy stabbing people had a problem? Or the guy who keeps getting contraband? Maybe they would use him? No. The Psychiatrist sits in his office to his lonesome all day, and I'm not really sure why I have him.
3; Criminally insane inmates just require a prison on top of a prison.
You are forced to have custom padded cells, custom padded solitary, and keep them to their own wing. I'm not a medical hospital. This is annoying.
4; If you happen to play on modes where traits are assigned as they spend time in your prison, be prepared for these annoying b*st*rds
I have 7 criminally insane inmates. None of which I wanted. 2 have escaped because my Guards refuse to do anything to them, and I don't have the budget to hire orderlies. This leaves me at an impasse where criminally insane inmates walk all over my system and ignore my rule, and meanwhile I still have to deal with high levels of contraband. This DLC isn't fun, the mechanics aren't fun. I'm not sure what objects it adds, besides a couple new rooms that only interact with the criminally insane! This is annoying because, this DLC effectively interacts only with itself. It's a layer of garbage, ontop of an already pretty enjoyable game. It's unescessary filler and clutter, and I hate it!

Do not release another DLC before you fix the bugs in the main game, and make the existing DLC more interesting and better.

So yea, this pack adds some nice new additions, but it really isn't worth $6. Wait for a sale, or buy it in the Total Lockdown bundle with the rest of the DLC, but it isn't a bad addition at all.

Create and manage a new wing of inmates with special needs and characteristics. Bring in psyche doctors, and keep the crazy less crazy.
AND MORE DOGGOS.

I'd advice any potential buyer against paying for DLC until the core game is properly fixed up to a playable state.
That said, There's some interesting gameplay here, albeit quirky and as yet glitchy.
The content here should be considered an extra challenge for a game; I'd advise starting out with it turned off, and then activate it later on when your prison is established and you want to add something new to the mix. Once you have activated it though, you can't ever turn it off again.

That’s it.
And I don’t mean that’s it in addition to all the normal behaviours for prisoners. That’s all criminally insane inmates do. They don’t work, they don’t go to parole, you can’t rehabilitate them which seems like a glaring oversight for a game that really tried to enable you to build the prison that suits your politics - punishment and rehabilitation in the base game need to be balanced or else you need to really understand the game and make extreme design decisions to get a functional prison heavily focused in one direction.
You can turn off intake of criminally insane prisoners, but unless you disable the option some of your existing prisoners will turn criminally insane over time. The game says this is due to their needs not being met but in reality I think there’s some sort of mean time to trigger in play modified by prisoner need levels. The practical result though is the same - a steady stream of criminally insane prisoners.
In smaller prisoners you can segregate protected custody inmates by schedule as well as physically, giving you a means to manage those tricky prisoners right off the bat until you can work out something a bit safer and more permeant. But the fact criminally insane inmates require their own special cells which are useless for anyone else means that isn’t an option - if you want to play with this DLC you can’t use it to just ‘spice up’ existing gameplay - it’s going to become a major part of your gameplay style.
On the surface this sounds good right? The only problem is needing to deal with special prisoners is already in the game and already far more developed. For needing to manage space and time to prevent interaction there are the aforesaid protective custody cases. How will you manage them? Lock them away for their own safety at the cost of building a specialised wing? Ignore them and likely leave them to their deaths? If you segregate them will they still be able to access reform programs or work? How many staff are you going to assign to their wing when there are only a few of them in there?
Protective custody creates lots of gameplay options and challenges to be overcome in a range of ways.
How about legendary prisoners? Unlike protective custody the problem is reversed as you need to keep everyone else safe from them! Like protective custody this becomes a balancing game of how much you are going to spend safely containing these Hannibal Lectors with the additional problem that they can just keep on killing prisoners and staff if you don’t manage them properly. At least once the protective custody cases are dead they are no longer a problem. And still there are needs, access to reform (or if you should even try) and lots of options to consider.
Compare all that with the criminal insane inmates. They turn up. They need to be given their own cells. They don’t work. They don’t really do reform. They are a class of prisoner with fewer options and gameplay mechanics than the rest of the population.
I just disable the whole thing when I play now and focus on the far more rewarding and developed base game prisoners.
And the bugs. I appreciate Paradox has inherited this game and as such will be nowhere as familiar with its nuts, bolts and guts as Introversion where but I’ve had two prisons now crippled by bugs. This sort of thing was more acceptable in early access especially for a game this mechanically dense but if I can have hours of work ruined by a truck that won’t move and can’t be dismissed you have functionally killed the game.
I was genuine excited about Paradox inheriting Prison Architect. I was looking forward to new additions and new prepared to pay for new DLC for a game I thoroughly enjoy and have on and off for years. It would be hyperbolic to say the game has been ruined, but Paradox really need to step up their game if this is a taste of what we can expect.

The premise seemed cool, to have new and more needy inmates. I expected to have to keep up with them a lot more, to have new traits, to stop them from mingling with regular inmates and them to have way more needs and NEED the special guards.
They didn't have any of these, all you got was a bar above their head, a new type of guard (which in fact you did not need) and a new uniform.
Being 'criminally insane' you'd expect them to be suicidal, banging their heads of the wall and Over Dosing etc.
DO NOT BUY

That being said, I am happy to announce that this game runs perfect on my POS. What a pleasant surprise! Usually there are games quite buggy for my system and some I cannot play altogether.
This game worked great.
Now the expansion, It really adds a new element to the gameplay. I really get a kick out of some of their actions, their write-ups etc. I get quite attached to some prisoners. XD
GREAT GAME! I recommend it.

The one thing that concerns me with many people is that they are quick to blame Paradox for issues they are having. I help a developer that works on a large scale 3rd party software project and I know through them how fixing issues work. Every computer can act differently to software, this means that one person can not have a single issue where others have have many problems with the same game even though they use the same platform type.
It important to remember that the people that work on these games are human too and rather than be quick to blame Paradox or the developers for your issues, just know that they will not be able to fix this issue for you or other systems that may have the same issue if you don't report it in some way.
I just want to point that out because I see many people pointing fingers at Paradox.
I have not had any issue with Cities Skylines or Prison Architect since they have taken over PA.

However, yes this is buggy and is it worth what $6 (£4.45) price tag? Just, yea. It does need improvement and lots of it but I think that it would come out to be a amazing addition to the game if paradox puts some effort into it. Some of the new prisoners are taken to cells but some are just left in reception instead of going to the padded holding cell.
Also the type of insanity means literally nothing for how you handle an inmate. Is your inmate claustrophobic? Does that mean they would need access to a yard or else risk heavy anxiety or panic? Nope, just chuck them into a cell with everyone else. If your prisoner is stoic and doesn't care about guards, does that mean they need extra security to keep them in check? Nope, they act exactly the same as everyone else.
Even really common elements of mental illness and insanity are left out. No criminally insane are at risk of self harming, ODing, or suicide at high levels of stress. Major Depressive Disorder and Generalized Anxiety Disorder - as far as I know - aren't even in the game. And while there are some nice touches, like insane prisoners making random piles of clothes, that's nothing compared to the lack of challenge or unique features.
Overall, I think that this game is bugged but has potential to be something big, all it needs is work paradox!
Get to it!

But this DLC has really messed up the game. Multiplayer doesn't work, and there are more bugs than there have been for years in Prison Architect. It's been close to three weeks since release, and it's still buggy as hell.

Firstly - and this is one of my biggest complaints - the type of insanity means literally nothing for how you handle an inmate.

And while there are some nice touches, like insane prisoners making random piles of clothes, that's nothing compared to the lack of challenge or unique features.
This DLC feels rushed. It lacks substantial changes or features. So far - it's six days after launch as of writing this review - the DLC is incredibly buggy, and I've had no shortage of headaches trying to work with insane prisoners. And beyond that, there's no reason this should be worth $6. It's less substantial than the free update that added female prisoners, and the only reason that it costs money is because Paradox is a disgusting, DLC-inflated leech of a publisher.
