Pathologic 2 is a narrative-driven dramatic thriller about fighting a deadly outbreak in a secluded rural town. The town is dying. Face the realities of a collapsing society as you make difficult choices in seemingly lose-lose situations. The plague isn’t just a disease. You can’t save everyone.

The plague is devouring the town. The chief local healer is dead, and you are now to take his place. You’ll have to look for unexpected allies. The local kids are hiding something. Try playing by their rules.

  • 12 days in an odd town ravaged by a deadly disease.
  • Time is of the essence: if you don’t manage it carefully, it’ll simply run out. You’ll have to choose how to spend the priceless minutes you have.
  • Survival thriller. You’ll have to manage your bodily functions, offsetting hunger, thirst, exhaustion, and so on. It doesn’t boil down to scavenging resources. Surviving on your own is hard; you’ll have to win over allies.
  • An uphill battle. Managing your bodily parameters may seem bearable at first, and as time goes by, it becomes harder and harder. Your own body is only waiting for an opportunity to give up and betray you. Things are changing from bad to worse and the odds are stacked against you.
  • A duel with an enemy you can’t kill. Your main foe is the plague itself, an incorporeal and malevolent entity that you have to defeat… without having the means to. It’s more powerful and more treacherous than you can imagine.
  • Loot, murder, mug, steal, barter, beg… or don’t. You need resources to survive, and it’s up to you how to obtain them.
  • The fights are short, ungraceful, and vicious. They’re not always lethal though. Many people—yourself included—would prefer to exchange their wallet for their life.

Among the most unique games I have ever played and a consistent oppressor of mine, Pathologic 2 is a masterpiece most people won’t like or rather, be able to endure.

When I say oppressive, I don’t mean to say it’s a survival horror game. The game CAN BE scary at times because you know it won’t really get easier and you will soon be taught your mistakes in this game have dire consequences. Nevertheless, it’s not a horror game(but an Immersive Sim, predominantly), you don’t have an Alien after you, there are no Necromorphs, you won’t be getting headshots on mindless zombies. Which means; getting better weapons(something possible to do) won’t make anything less stressful. I’m not sure if you can get through the game without killing if you want to finish every quest but your kills won’t be very numerous. What is scary, is the stress of having an entire town in your responsibility. In this context, Pathologic 2 is anxiety inducing in so many ways.

Because on the contrary to most games, you main shtick in the game is not killing, but saving people. You are a young surgeon, Artemy Burakh, back in his weirdly supernatural hometown after being called back by his father who is also a doctor. Soon after your arrival to a not-so-warm welcome, a deadly plague erupts. A plague with a mind of its own, a fatal disease that affects different parts of the Town each day. You have 11 days to save our ancestral town and believe me when I tell you, it won’t be easy.

Those eleven days, are obviously in game days and not actual days but they are not the type to wait for your sorry ass. The time flows and the sun won’t wait for you to finish your quests so it can hide behind the mountains. Events won’t unfold lazily just so you can follow them in their entirety. People won’t stay alive just because failed find a cure for the disease and you can’t take the glory just because the game catered to your need to be victorious. I mean all video games, even this one is made to be won but you have to show REAL effort for Pathologic 2. Time is your biggest enemy unless you learn to utilize what little of it you have and properly because you have to take the good with the bad in here.

Still, Pathologic 2 is a flexible game. Almost every named NPC in the game can get sick and die if you’re neglectful, forgetful or simply short on supplies. The game itself will still stay alive and progress, you can still witness all 4 of its exquisite endings by way of reloading the proper previous saves. BUT if you just sit there/sleep in order to skip days, you yourself won’t stay alive as Artemy, like a normal human being, needs sustenance. You are a hungry, thirsty, exhausted organism all day, every day and your in game character isn’t really different. The food in this game is expensive and it gets more expensive as you progress because there is hyperinflation due to the epidemic. You have to be wary of plague clouds, infected people, muggers and all the other shit that hate you, devise a route to make your wanderings in this labyrinthine town as efficient as possible and try to keep your character fed in the meanwhile.

Why should you care about this though, what happens if your character dies? Do you lose the game or do you just get sent back to your previous save? The answer to this question is not simple. No, you don’t lose the game, you will be revived by a certain character, talk to him about life and shit and then get sent back to your last save. Yet, a little time is not all you lose; your deaths will give your character permanent penalties. Your health bar will be reduced bit by bit, your hunger meter will fill slightly faster, you will get thirsty easier and exhausted more quickly, etc. So the game won’t take pity on you, if you don’t play carefully it won’t pat you on the back and say “you can do it!” It will just hit you harder and become more opressive. Reloading won’t take these penalties away either, because they are saved on your character profile. If you really want to savescum, you have to take manual backups of your saves.

Let’s add things together. The game is really hard and it will get harder even if you manage to not die at all. It does not give you shiny loot, insincere compliments or use any other of the boring tactics games nowadays usually use to inflate your ego and make you secrete that sweet sweet dopamine. It will also progress no matter who dies, as long you manage to keep yourself alive. It’s not “fun” in the general sense of word and it’s an uphill battle. So why should you bother with it? What do you stand to gain other than the bragging rights? The answer is: a delicious fine dining on Pathologic 2’s story. Pathologic 2’s narrative is a puzzle whose pieces are its characters. Not many of those characters you will love or even like, as even the children in this weird town are creepy. Yet, all of them will provide insight into the true nature of this fever dream of a town and the oneiric events unfolding in it. Pathologic 2 is an exquisite piece of weird fiction and is like no other, no other that I know of at least. Is it Lynchian? Nope. It is Lovecraftian? Nein. Is it Mievillesque? Nyet that I think so. It has an atmosphere and mythology of its own you will really enjoy discovering. If you are addicted to good narratives, there is a good chance this alone will be enough to give you the drive to save this Godforsaken town and its residents. Your successes and failures in doing so will comprise the bulk of the emotional impact Pathologic 2 will “bestow” upon you.

PS: This game is a remake of the 2005 cult classic Pathologic. The reason it’s called “Pathologic 2” is it being not just a simple remake with better graphics but an extensive reimagining, both in terms of gameplay and narrative. The original game apparently had 2 other playable characters with stories of their own, Bachelor Dankovsky and Changeling Clara, both of whom you meet in the game as Haruspex, Artemy Burakh. These characters will have two additional scenarios added to the game in the future but apparently things like Russian-Ukrainian War and Pathologic 2 not being a commercial success slowed down the development on those. Ice Pick Lodge is a Russian developer that neither supports nor condones their government actions on Ukranian soil. I’m not sure if they receive payments for Steam sales right now due to sanctions but I support the studio and wholeheartedly recommend this game to anyone who won’t be pushed away by the harshness of its intended difficulty. You can lower the difficulty, but I don’t recommend it as it takes away from the first time experience.

I remember my first time playing fallout new vegas, it was a surreal experience which I thought nothing would top, I am very much into rgp games where player choices matter, I love the feeling of having to make decisions that will have an actual outcome depending on what I choose, I thought fallout new vegas did that very well... very well until I found this game.

I've always wondered what made games where you have to make decisions in good, what really made me like fallout new vegas so much from other games apart from the amazing quests, other games had fun quests with really good characters but there was something about fallout that I couldn't quite put my finger on, I just couldn't until I got to play Pathologic 2 and as soon as I did, I realized that having no obvious image of which decision is right or wrong changes how you look at a game entierly, it is something pathologic 2 does better than any other game I've ever played but this is just scratching the surface, not even close to what makes this game amazing.

The game is intentionaly misleading, it plays on what you believe is good and bad very well. It changes the normal formula of story,world and character building and in exchange creates a whole different way of thinking, it forces you to think in a way that you are not use to, questioning what is actually right and what is wrong, who you can truly trust and who you should be suspicious of, it gives you the feeling of satisfaction when you figure those things out and believe me it is not easy.

For the newcomers to the series of pathologic, plot and certain dialogue interactions can be extremely confusing, however as you progress everything will start to become more clear. I am only 4-5 days into the game and I can not wait to make more progress to get more of the story.

I'd write an actual good and detailed review if I knew how to put into words how much this game means to me and how much it has affected my life. It's a good game, play it. Especially if the theatrics and incomprehensible dialogue and suffering-based gameplay of classic HD is too much for you.
Just be prepared to fall in love with a fictional organ harvester, cry over several fictional children, and either want to punch or kiss a badly dressed thanatologist.

This is honestly an incredible piece of media. One of the few times in recent years when I felt that a video game was a actually worth my time, instead of a waste of my time.
What you get is a overwhelmingly tense atmosphere, a unique and well-told story and some of the most memorable moments in gaming. The sense of despair the game makes you feel by not allowing you to save everyone and finish every quest is something you hardly ever experience in other games.
It's not for everyone – but I definitely suggest you give it a try. I also recommend turning down the hunger meter to make the game less frustrating.

One of the very few games I've played through twice. It sits currently in my legendary category of games, a category of video games that transcends the conventional genre and elevates the medium as a whole.

I can guarantee that Pathologic2 will be unlike any other videogame you've played. The buildup with the characters and the sacrifices you end up making will really stick with you.

That being said, it’s brutal and not for the light hearted. You WILL struggle (and that’s a good thing) to survive. It is a very intense game and shouldn’t be taken lightly. One should play Pathologic2 for enlightenment and to embarrass the inevitable challenge that is life’s struggle on something memorial and worthwhile.

I'm going to be honest up front-- this game is not for everyone. It is slow, and frustrating, and can be very punishing. The game itself seems to wave a war against you, and at times it feels like it is actively antagonizing you... So when you finally make it through, its all the more satisfying.

It's not a super long play, all things told. Ive played the game through twice in succession, once on the easiest and again on the hardest settings, and it was a unique and rewarding experience both times. If it piques your interest, i highly recommend playing. The game has a lot of great things to say.

No game has ever made me work harder to earn my happy ending. Also runs like absolute ass so the suffering is twofold. It's less like playing a game and more like being a character in a Greek tragedy in which your suffering is a parable. I'll never experience another work of art like this. So give them your money.

An artistic achievement. Just one of the three campaigns so far, but I trust IPL in delivering the other two- seeing how insanely much content the Haruspex holds alone though, it's more than well worth the price already. Difficult, emotional, genuinely immersive, and absurdly full of niche elements I personally enjoy (inventory and time management and harsh survival mechanics). Genuinely the best game I've ever played.

I keep dying of hunger, infection or stabbed by the people I'm desperately trying to save. Death jumpscares me in my home and flirts with me. I've adopted 7 kids that will probably die before day 12. I can't even hook up with the Bachelor. I've never felt so stressed just for walking around a city. I still haven't found a cure for the plague and I have no money, food, or medicine left.

And I'm having the best time of my life, I absolutely love it.

EDIT: All quirky review aside, I feel like I need to add something, because I beat the game and its two endings since then.
It is, in a very strange and peculiar way, the greatest game I ever played.
It will haunt me. Stay with me forever. I have this weird feeling of loss, because I know that there will never be another game like this one, that I will never experience again what Pathologic made me experience.

As I saw someone else put it, this game is not for everyone, but if it's for you, it'll be the best game you'll ever play.

I'm not clever enough to know WHY this game had the impact it did on me, but I know it had one of the greatest impacts a game has had on me, ever. It has made me think about myself and the world in new ways, and for the 20hrs I sunk into its completion, it was at the front of my mind for all of that time. It does something to you, this game. I haven't completed it yet but I can already say that this game is great and different to everything you ever played. It feels hopeless, chaotic and janky at times. The story is good and intresting, but also creepy, cryptic and depressing. If you ever wanted to play something special, horror that doesnt use jumpscares but gets into you by gameplay mechanics and story, try this out. But be advised that you should get into this game as spoiler free as possible!

Don't mind the playtime. I've spent 100+ hours on this gem on Microsoft Store and bought it again on this sale because the game deserves it.

I can start saying this has become one of my top 3 games ever after almost 15 years gaming actively. Other than that, I think it's like any good piece of art: When you try talking about it, you notice how useless it is to try and put into words; they lose meaning pretty quickly.

I can only ask you to give this experience a try:

-Fantastic storyline with unprecedented depth and mystery.
-The feeling of being in a living russian tale where, in true kafkaesque fashion, the absurd is transformed into commonplace, which is uniquely terrifying. You never understand what's up with this world or its people, and in an attempt of understanding it you almost go crazy with them.
-The fact that you're always miserable and almost dying gives an urgency that you will not find in any other game. You will be rewarded by your resourcefulness, and being smart when trading and exploring.
-Time is precious and you're forced to think twice before just going on a stroll through the city. You have to have a plan, which is absolutely terrifying when there's no right place to go or right thing to do. You're truly on your own while multiple different conflicts are getting progressively worse and more tense with no real solution in sight for any of them.
-There is a deeply tribal/spiritual vibe to lots of themes in the game. It overlaps the two and feels like a fever dream many times.
-The game is full to the brim with beautiful symbolism and esoteric themes that are very rich and original, you can't really trace (at least I can't) the inspirations for the philosophical and metaphysical ideas presented which opens a completely subjective and personal interpretation; a real trip. Expect to ask yourself a lot of things.

What is the Polyhedron after all? Is this city doomed? How could things get so bad so quick? Do you actually have agency or is fate inevitable? Is this story an opportunity for your character or an example of absolute absence of opportunity? Is civilization and nature ultimately incompatible? How much can we expect to disregard nature until it repays us for our stubbornness? Is it even capable of doing such a thing, or is our ignorance our self-destruction? Are the native people's occult-esque forebodings a true explanation of what's happening or is it just symbolic nonsense? But what is real in such a screwed up world where nothing really makes sense?

These are a few of the many questions the game will throw at you. Even though I developed a deeply personal connection with what I took from it all, you really won't be able to give a compelling answer: Just like in life, you live it, understanding it perfectly or not. You may get to the end of it, see the outcome of your actions, and still be riddled as to what it all meant. Was there an overarching explanation or lesson to be learned? Is there a metaphor to be found? Maybe. But you can be sure of one thing: you've learned a lot.

More then just a game. Definitely best game, developed by Russian studio.

Main pros:
- Solid story with high role-playing variability, mysticism, plot twists and meta-thinks
- Exact inheritance of the first part (story, environment, characters) which is awesome
- Sweet visual style
- Atmosphere... Whole city collapse into the abyss
Main cons:
- Still hard as hell (you can barely survive another day), high entry threshold 'cause of it
- You won't understand anything if you not played Pathologic HD, which was released in 2005
- Bad perfomance: game lagging alot (right after loadings, when you enter new region etc) even on cutting-edge PC
- Excessive theatricalization

Pathologic 2 is art house. You should be ready to specific aspects of this game.
But it's worth a try, for sure, 'cause you never played anything like this before, I can guarantee. Definitely recommend.

When I’ve sat down to play games that promise me that I’ll craft my own story and experience them differently each time, I’ve been disappointed. BioWare really cares to set out one or two paths, particularly in its Knights of the Old Republic where your story is either one of an unbelievably squeaky clean Jedi with a secret or an unabashed asshole. You’re going through the same quests in the same ways with dumb dialogue trees. David Cage games, ignoring their silly writing, do have genuinely branching story paths, but Detroit: Become Human betrays the player by being laid out openly on a pause menu as you go, so the mystery and ingenuity evaporates even if you can tolerate the dialogue and messaging. JRPGs tend to have a lot of character customizability options, and I guess a game like Pokemon technically has thousands of different ways to experience it, but you’re still hitting the same dramatic story moments and overwrought sections about killing God no matter who you’re dating.

Pathologic 2, I’d argue, is the first narrative-driven game where the promise of a new experience each time is truly fulfilled. Beyond that, this story isn’t delivered like a book or a movie but specifically through the language of video games. For those reasons alone, it’s one of the greatest successes in the medium to date.

Pathologic 2 is, broadly speaking, a first-person survival horror game. You manage a few statuses—your basic health bar, immunity, exhaustion, hunger, and stamina. Your management of these statuses precludes you from or allows you to access new story content. Over the course of twelve in-game days, you’ll balance these with your core narrative duty: the investigation and ultimately cure of a deadly pathogen that is afflicting the Town. The game builds on great survival horror mechanics where careful inventory and time management are key, but it is particularly unforgiving. Statuses drain very quickly and, with the exception of health, automatically just by participating in the game. Unlike something like The Last of Us, you’re not guaranteed to get pickups for the weaponry, food, or other various items you need in certain locations. There aren’t quest rewards like in an RPG, except in the very beginning. Most of your item acquisition will come in the form of bartering with the Town’s citizens, though you can also kill some of them for a chance at getting some essential items.

This is the central tension of the game made explicit: do you pursue story objectives and garner respect by bartering at a loss so that you can do more trading later, or do you ruthlessly pursue your own survival and stock up on essentials through robberies, muggings, and murders? You’re genuinely free to choose, and you’re not going to be railroaded into a “good” or “evil” ending. Your morality isn’t reflected by a blue and red bar, just by your own gut. You can completely ignore parts of the story you think are a waste of time, and they’ll just happen without you present. You could, in theory, reach an endpoint in the game by simply surviving for the twelve days and doing literally nothing else. This is a cool subversion: in typical games, your survival is given in the context of a story. If you die, you restart at the last checkpoint or whatever and continue the story as-is. In this, not only do you get lasting consequences for death, but it’s really just your survival day to day that’s given by the story. The beats don’t care at all about you.

This is the kind of genius sandbox gameplay that isn’t captured by big flashing quest markers in an open-world game. These tight, mid-level maps in this, Dark Souls, Disco Elysium, and others are absolute dynamite for me. They serve as a perfect balance of player freedom and designer intent that makes for some truly staggering depth. Part of the magic of all three of these games is honing player mastery not through the difficulty of twitch mechanics (though those exist in this and Dark Souls in their own ways, of course) but instead in familiarity with the lived environment.

Pathologic 2’s light use of quest markers distinguishes it from Dark Souls and is cited in interviews as a compromise that the development team felt they had to make very late in the game’s creation. For a game like this, it would probably make the most sense to have the player interface with a diegetic map and have to plug in their own knowledge, maybe mark it up, to find quest NPCs. I think that that might have worked effectively, and I’d love to see if the team decide to go in this direction for the installments featuring the Bachelor and Changeling. But I will defend quest markers in this game briefly in that they totally subvert the standard usage of quest markers. Where those little icons are typically luxuries, auto-pilot way-points, or just a mound of distraction, they work here in a fundamentally different way. The icons on Pathologic 2’s map taunt the player: do you know the map well enough and do you have the savvy to navigate to this place you really should be right now? It’s not rare to find yourself going to one quest marker in one corner of the map, only for an NPC to rush up to you and insist you must make your way to the opposite corner for another story event. Someone managed to make convenient map interfaces into something that’s stressful. That’s pretty genius.

And it carries over to the other holdover of lesser games that Pathologic 2 indulges: dialogue trees. Pathologic 2 uses a strange, half-obscured language and cadence that immediately instills a sense of mystery to the proceedings. Without full information and context, the player’s decisions in dialogue can be somewhat rushed, clumsy, or random early on. Even by the end, there won’t be clear “good guy” choices and “bad guy” choices. What’s really fascinating about the dialogue trees is that rather than serving as simply a way to move the plot along—though they do serve that function, too—they serve as check-ins to see how much more of the world you do or do not understand. This is an excellent mystery story, where people’s intentional lies and deceptions will only be seen through by the aware player. Can you cure this disease and find your father’s murderer? You’ll have to follow the right breadcrumbs and develop a genuinely sharp in-world intuition. And again, these dialogue choices don’t artificially lock you onto certain story rails but instead just are the narrative as they happen. It’s really nuanced and expert deployment of a system that seems to only be improving in quality of writing rather than quality as mechanic in recent years, and I respect it a lot.

And that’s really what the whole game comes down to: some pretty well-worn ideas, some dated and maybe even janky mechanics, but put together with so much love and so much genuine insight and genius that the whole becomes this magical, engrossing masterpiece. For the 30 hours or so that I played the game over a couple of weeks, I had a hard time focusing on much of anything else. The game is truly exhausting, and I couldn’t play more than a few hours a day, but the non-playing hours were filled with plot speculation, a sort of Tetris effect of running through town trying to find empty bottles and robbers to beat up for beef jerky. And the wonders that it unravels as you begin to dip beneath the surface and find what’s truly going on in this Town are some of the most staggering I’ve seen in a recent video game. Highly, highly recommended.

Para gostar de Pathologic, é necessário esquecer de literalmente tudo sobre video games de sobrevivência em geral, ou a experiência vai ser frustrante. Antes de descobrir isso, eu detestava o jogo com cada pedaço do meu ser:
Não é um jogo sobre ter uma barra de comida cheia, é um jogo sobre passar fome. Não é um jogo sobre triunfar, é um jogo sobre falhar. Até você entender que toda a desgraça é uma mecânica e não a culpa do jogador, você não vai entender o jogo. A inevitabilidade das desventuras simplesmente o completam.
Mais da metade que jogou esse não o terminou, então se eu disser na sua cara que você não deveria jogar e você comprar do mesmo jeito, então talvez, e eu duvido disso, você tenha o potencial para
suportar a desfortuna da praga e chegar no fim dessa desgracenta mas linda experiência.

This game is not fun, and that's the point. It's an endurance test.

You roleplay a doctor on a fool's errand to try to save a doomed town. This isn't a power fantasy where you rack up skill points, conquer your foes, and save the princess. It's a brutal existential nightmare where your enemy is unknowable and everything in reality will try to stop you from doing your duty.

To put it shortly: The combat sucks, the survival mechanics are unforgiving and frustrating, and the stress of time management is overwhelming. The writing is loopy but great, the atmosphere is fantastic, and the music is some of the best in gaming history. To really appreciate Pathologic 2, don't think of it as a fun gaming experience: Think of it as the game asking what YOU would personally do in a devastating crisis, and testing to see if you are willing to persevere as the good doctor. I think that this experience is something that few games can offer and everyone should give it a try.

The premise of the game is that you're a doctor in a town that's been infected by the plague. This game isn't just hard, it's straight up unfair to the point where sometimes you'll fail regardless of what you do. The survival mechanics are brutal, and I found myself ignoring quests just to focus on keeping myself alive.

The game is genuinely frustrating. But there's a purpose to all of it. In most games you play as a hero, someone who's able to fix every problem. In Pathologic you play as an actual part of the town and you take on their problems. It creates a genuine sense of empathy when you hear someone talk about how desperate they are because of course, you're just as desperate. The mechanics of the game don't just put you in the role of a doctor, they put you into the actual character that you're playing. It's so rare to have a game where the gameplay supports the setting and themes so strongly. I'm struggling to think of anything that's quite the same.

From the discussion online I've read a lot of people saying that the game isn't for everyone. But I think that's a disservice to both the game and the audience. The game requires you to trust the developers that you'll get something out of it, but I do think anyone could play this and get the experience they intended. If you care at all about video games as an art form you should play this.

after seeing a youtube video recommending this game and the in depth aspect and what is great about the first. I thought it was worth trying even if it is not for me. it is a horrible experience and one i couldn't recommend enough. this is so far out of my comfort zone for a game id buy and try . but i am glad so far that i have. this is something i definitely thought would not be for me and i have been proven wrong. So far, i know the worse is to come and i am sucked in so much now i don't know if i will hate myself more for trying to finish it. or for admitting defeat

I don't usually review games, but I do consider Pathologic 2 to be one of the most underrated games of all times. Hence, I'd love to help it get more recognition.

The game takes place in a small, unnamed town somewhere on the Russian steppe. You play as the surgeon, Haruspex (literally the person able to foretell the future by inspecting the entrails of sacrificial animals) named Artemy Burakh who returned to his hometown after receiving an unsettling list from his father. I won't spoil the plot, but pretty soon a nasty plague called sand pest hits the town and as a doctor it's your task to help the people in need, and ultimately find a cure.

Easy enough, right? Wrong. The game has survival mechanics, such as hunger, thirst, exhaustion and immunity meters. Thankfully they are not as annoying as in other survival games (for example, two big meals will usually be enough for a whole day), BUT, often filling one meter will slightly lower another one. You want to eat a salty fish? It's filling, sure, but prepare to drink water afterwards as you'll be quite thirsty.

But how do you get food? The easiest method would be to buy it, right? The problem is, the food in this game is extremely expensive and while it is possible to afford it from time to time, the better idea is to barter with people. Various people want various things. Guards for example will often carry toasts with them and would gladly take your lockpicks and weapons in return. Women often trade for things like spindles or thread and needles, but the best part are children! These morons would gladly give you the best painkillers for a chestnut! That's why you're much better off finding marbles and random nuts than trying to get the money.

This game is really immersive and full of moral dilemmas. Do you kill innocent people to get the stuff you need to survive? Do you keep immunity boosters to yourself, or do you try to save one person from dying? Do you go to sleep to recover some health, or do you risk dying yourself? Do you go to the infected districts to get supplies, even though your immunity is low already? Do you go to that one dying person far away, or do you try to save multiple people nearby? Basically every minute you'll be making tough decisions, each one having consequences. Almost every NPC in Pathologic 2 can die, almost everyone can be saved.

Oh, did I mention that you don't have infinite time to find the cure? You have 12 days, each one lasting about 2 real-time hours. Every day, the town changes. Every day, people can die. The time passes all the time, and the only way to stop it is to pause the game or talk to NPCs. Hence you often won't be able to do all the daily tasks the game throws at you, and will have to prioritize. Every now and then you'll have to sleep or you'll get exhausted, that further down reduces the time available to you daily.

And well, since I didn't want to spoil almost anything regarding the plot - you'll have to believe me it's really well-written and interesting. Also, the aim of this game is to make you feel stressed and hopeless all the time, hence it's definitely not a game for everyone. It has adjustable difficulty settings, but even lowering everything makes the game really difficult (or so I've heard - I've completed the game on the hardest - intended difficulty and would also recommend that). You may not succeed on the first attempt, you may find yourself in a death spiral you can't escape, and every death takes away a chunk of your maximum health (EVEN if you load previous save), but it's definitely well worth it trying to endure for this story and immersive world.

What an experience. It's totally possible to get both endings on intended difficulty, just take your time and don't be afraid to start over.

An excellent game presenting a unique experience in the world of gaming. I've never played anything quite like it before.

Pros:
+ Immersive soundtrack, characters, plot and level design. Every area is surreal in its own way.
+ The game gives you a sinking feeling that gets deeper and deeper as time goes on. In parallel, you learn how to manage the near-doomed world you play in, and try to combat the lack of time and resources with experience. Just when you think you've got the hang of things, something else goes wrong. It's great that way. There's always a bit more than you can handle.
+ Time and death are treated in a very interesting way, but I won't go into detail so as not to spoil.
+ Great depiction of the cultural clash between the modern inhabitants of the town and the more mystical people of the steppe;
+ The tutorial is pretty good and shows up at appropriate times;
+ You will be surprised in some way every ingame day, even after you think you know everything.
+ The barter economy is actually pretty cool once you get used to it. Almost addictive towards the endgame.

Cons:
- Combat is kind of rough, and on the intended difficulty some sections are unfair to the point of just being unenjoyable, even if you're prepared with a revolver.
- The game crashed for me on occasion, which is especially painful given you can't save at will. Mostly stable, though.
- A lot of dialogue options felt completely irrelevant, and the character would still say almost the same things no matter what you picked. A bit immersion-breaking.

Overall, a great hidden gem. Not for everyone, but if you want a pretty hard challenge in a surreal environment, this game will not disappoint.

Please, be careful. There is not too much time left.

Victory is impossible, suffering inevitable. You’re not dying for love, or for freedom. You’re just dying, again and again, each time with more dread, more pain, and another gaze into the existential abyss.
You’re going to have to live with your mistakes. Things will go wrong, often outside your control. You might want to try to be the hero, but, please, let’s focus on staying alive. Being heroic is only possible when you’ve got the means to be heroic, right now, that’s not within your reach. Your most valuable possessions are your organs, and you won’t be worth them.
Making it to the other side is only possible though an act of rebellion against the absurdity existence. With the position you’re in, it would be foolish to endeavour helping anyone, foolish to go out another day and attempt to save someone, foolish to even try.
You’d be like Sisyphus, pushing up the boulder, again and again, only to watch it roll down, and force you to start over. Only a fool would subject themselves to that.

But, hey, wanna be that fool?

this was one of the most unique gaming experiences ive ever had.

where to begin? lets start with what to expect. the game is brutal but incredibly engaging. you may need to sell guns just to get a piece of toast just to keep going, this sounds awful AND IT IS! its great. the whole point of the game is to be painful. what keeps you going is the amazing story that keeps unraveling. 10/10, never forgetting this.

GO ONION, I NEED TO COLLECT THE TOOLS!!!!!!!!!!!!!

This game is a good pick if you a) have heard about the original pathologic and aren't sure if you'll enjoy it or b) like video games that are good. It isn't fun, it's horrible and stressful. This game has given me real anxiety attacks and stress nightmares. But it's got some of the best story telling in any game that I've ever played. And it uses it's gameplay to tell that masterfully tell that story.

In this game you play as Artemy Burakh- The Haruspex and also the hungriest motherfucker on the planet. I won't go into the story because all you need to know before going in is on the store page, but the gameplay is actually very engaging. There are a lot of different ways to play and go about surviving, I really enjoy it.

There are difficulty settings, but even if it seems impossible, I think everyone should get through the game on the intended difficulty first. (The game makes this clear, but I feel like it's something I should mention)

Also- Yes there's only one of the 3 stories out right now, that doesn't mean the game is unfinished. This is a full game. One play through is about 40 hours. The other stories will come but I don't think you should wait for them to buy patho 2 (Buying this game will actually help them come out faster!)

I have no idea what's going on and I'm freaking out about everything that's happening. I don't like this town and it's weird layout and OHMYFMGS what is that thing coming for me!! Cool, I'm allowed to loot the infested houses... WAIT, that's not safe at all!

I just want to find some food and stay safe, perhaps cure someone or harvest some organs.. Atleast I kept my fathers house, I think. Or did I? But only after raiding another childrens stash. I hope the floating fortress starts crashing down at some point, or perhaps I don't want that. I'm not sure yet.

The crows... Gotta watch for the crows... Why am I already so exhausted? I have so much more to do! Oh well, gotta watch another theater rehersal. Or do I? I'm not sure. Still going to do it. Just in case.

Pathologic 2 is fun the same way that amputating your foot to save you leg is fun. I can only recommend it to somebody who is willing to sit through hours of emotional abuse.

This game is dark, oppressive, difficult, unrelenting, stressful, morose, and one of the most engaging experiences I've had in a long time.

Pathologic 2 is a game unlike any I've ever played. Tight and generally polished from a technical perspective, but not easily described as a game in the traditional sense. I'm struggling to write a review but I feel compelled to do so after experiencing it, and if you've read other reviews you might know why.

There's this feeling of dread while you're playing, like you're stuck at the bottom of a sinking ship and with every floor you manage to ascend the ship sinks further. You feel helpless but oddly driven to carry on, each day adding to the immense pressure you're experiencing but also promising a slew of new answers and questions as you fight to save the city. The passage of time is what makes this game so unique, it looms over you with every passing minute. Every day you will struggle to survive, every day you will make decisions that have consequences far beyond what you originally anticipated.

There's so much I could say but this is something you should really experience for yourself. Go in blind, let whatever happens happen. I think I'm going to come out and just say it.

This game is a masterpiece, and you should absolutely play it all the way through at some point. It sticks with you, it might just stick with you forever.

Despite being praised in Russia and having very positive reviews on Steam, Pathologic 2 didn’t get much traction upon its release in the west, with “game journalists”, a term I still don’t fully understand, comparing its difficulty to Dark Souls (yeah, some people still use Dark Souls as the litmus test for game difficulty) and claiming that it has Skyrim-inspired RPG elements. It’s as if “game journalists” have never played anything outside of Bioware games, Skyrim, Dark Souls and Pokemon.

The game takes place in Town-On-Gorkhon, an isolated town in what appears to be Siberian-esque steppes. From a glance, the town might just look like your average early 20th century Russian town, but it’s inhabited by two groups of people: the Townsfolk, who are just becoming industrialized, and the Kin, a group of Steppe nomads who hold veneration for bulls because they believe that the town rests on the back of a giant auroch, Mother Boddha. In addition, the latter group has a species of humanoids called Worms who water the ground with blood to grow plants, women called Herb Brides who dance in the steppes to make the twyre bloom, and other practices. Despite the contrasts, the two are not at complete odds at each other; rather, both cultures have meshed together.

Pathologic 2 is like an adventure game and a “horror survival” tied into one. The imagery of the game goes from uncanny valley to flat out dark, with red pustules and moss-like substances growing on the buildings and streets of infected districts, infected townsfolk shuffling towards you to try and infect you, and plague clouds that manifest and chase you down the street. If you’re unfortunate to get infected with the plague, you hear voices in your head telling you, gently, to lay down and die so your suffering can cease. While you’re trying to find a cure and trying to save NPCs from the plague, you yourself are trying to survive.

Your overall survival is dependent on more than just your health bar. Sergei tries to shank you for your track suit and Semechki seeds, yeah, your health will go down if he manages to hit you. But then you have to factor in your hunger, exhaustion, immunity, and stamina/thirst. You’re hungry, so you eat some toast, but now your thirst meter is going up; while it’s not immediately detrimental, it affects the duration you’re able to sprint and fight. Your exhaustion meter is full, so you lay down to sleep for a few hours, but now your hunger is going back up and you’ve just spent precious hours that could have potentially have been used doing something else. Uh oh, you just got hit with a plague cloud and your immunity is dropping - do you use the immunity boosters/tinctures you were saving for patients to bring it back up, or are you going to take the risk and wait for it to slowly climb back to where it was?

Any time you die, your screen blacks out and you speak to Mark Immortell the Theatre Director, who gives you a tut-tut-tutting on dying and sends you back to your last save file with a penalty. Your maximum health/exhaustion meters are reduced, you get hungrier and more tired as time progresses, so on and so forth. These all stack, and they’re all permanent across all save files, so there’s no going back to scum save to prevent the penalties..

This ties back to my previous statement about how people were criticizing this game. A lot of survival games in modern gaming tend to be generous towards the player in terms of, well, survival - you have a meter that’s running low, or a supply that’s dwindling, so you stop whatever you’re doing to rectify the situation. Should you fail there’s usually an “out” by returning to a previous save. You can’t do this in Pathologic; one reason being mentioned in the previous paragraph. Another is the fact that time is always working against you - really, the only moment where time tends to stop moving is if you’re in a dream sequence or if you physically pause the game. You're always on the move.

The game has a lot of choices, and not in Peter Molyneux’s Fable or Black and White perspectives of “choice”. The decisions can vary greatly. One of your friends needs a water barrel because they want to get water for the poor and impoverished in their district. Well meaning, but what if it infects the neighbors? The hospital needs the tinctures you need to boost the immunity of nameless patients; everyone will like you more if you carry the task out, and you’ll get paid the next day, but what if tomorrow means that half a dozen cast characters get infected and you don’t have the time to make more tinctures?

Critics of Pathologic 2 have bemoaned the fact that you can’t just walk around, immune to virtually anything and everything, and talk to the NPCs while freely exploring the town to learn more about the Sand Pest and the overall story. The desire to know more about the story is a fair point, but here’s where I see the problem:

The difference between Pathologic 2 and games like Firewatch, Dear Esther and What Remains of Edith Finch is that the latter group takes a more “hands-off” approach in their storytelling. You don’t have a lot of interactivity or mechanics that directly tie into the games. The named NPCs you speak with in Pathologic 2 are fleshed out; it’s personal because Artemy Burakh has history with them, and the decisions that you make, or don’t make, will ultimately decide their survival. Many of them have multiple outcomes; you speak with them, see their angles, see what information they may be willing to give out or abstain from initially giving, so on and so forth. The game pushes you towards investing them emotionally. Not only are you trying to save them from the plague, but you’re trying to save yourself. Rather than watching a sinking ship, you’re part of the crew trying to bail the water out and plug the hole.

Not all the mechanics are perfect. Guns and their ammo, while being extremely rare/expensive to find, have a tendency to jam up way too much and hitboxes can be choosy. Hand-to-hand combat can feel clunky, and the inventory can be a colossal pain in the ass to manage since the game does not auto-sort individual stacks and uses Diablo-style inventory management. However, I have very rarely seen things like these critiqued by the “video games are art” crowd; rather, they complain about the meter management. The problems of the town seem real because you’re in it as well. Without having to manage your meters, making sacrifices and decisions, it takes away the conditions that make moments in the game memorable.

Remember: Failure is a very real, understandable and relatable aspect of human life and society. There are times in life where you fail repeatedly before you see the light at the end of the tunnel and triumph. One of the marketing pitches of this game was, “You can’t save everybody”. For example, I spent three consecutive days treating Andrey Stamatin after he was afflicted with the Sand Pest, and it ultimately came to naught because he died anyway. Some of the game’s most memorable moments and interesting dialogue come when you are unsuccessful, because the game knows that you’re going to fail at some points even when you try your best.

Overall, would I recommend Pathologic 2? Absolutely.

Would I recommend it to someone who cares about story-driven games? Totally.

Would I recommend it to people who have low frustration walls? No.

This game is absolutely insane - and i mean that in the best way possible. It isn't afraid to push you to your limits and impose as a challenge for the player, as most of the time you WILL be struggling to survive / get everything done. Time is not on your side and you can feel it with ever passing day getting harder and harder, but in a way, It makes everything seem more real. Pathologic 2 is not going to give you an easy win. If you want to make it to the end of the game you have to be prepared to crawl on your last leg after being tossed through hell. The characters, music and over all atmosphere make a unique experience that is hard to come by. And the story? Don't even get me started. The amount of twist and turn you encounter and the deep meaning behind it all ... Do yourself a favor: play the game and understand it for yourself.

Yes the game is difficult, but pushing yourself to make it through it makes all of your efforts worth it.

A masterclass in interactive storytelling. As a good friend once explained, Pathologic 2 is basically a playable tragedy act disguised as an RPG with survival mechanics.
A equally gorgeously beautiful and horrifying distressing world filled of attention to detail enriched by it’s unique ancient folklore, inhabited by all sorts of characters with their own goals and outlooks on the deadly, horrible plague that is decimating the population.
In it’s intended difficulty experience, this game is nothing short of dread fuled, exausting, borderline upsetting moment to moment decision making nightmare, dangling between keeping yourself alive with any means nessesary and doing everything in your limited power to find a saving miracle to your home town.
Everything you might heard is true, it won’t hold back. You’ll have to improvise, pay attention to forshadowing, have long and short term memory, make a routing strategy, think on your feet and carefully consider your next move. Or not because suddenly a new curveball is set up for you to deal with and adapt on a whim.
You will get wrist slaps all over, trying to uncover the dark secrets of the town, saving as many as possible, seek answers to the otherworldly elements that sorround your people, or die trying.
Respect the game and it’s rules, and it will respect you back (admittedly spitting his hand before the shake) with a phenomenally well told story which rewards further due to the struggle involved to see it’s conclusion.

A dark mixture of survival horror and RPG where you are in a setting of a town with a plague, and as a doctor, is tasked into saving the town as well as yourself before things get out of hand, which starts to happen quickly. The dread in the game enhance well with the atmosphere, environment as well as the music. It is also interesting that there is a finite amount of resource, so the game then gives you a push into doing activities such as fighting and trading with others in the game, which adds to the survivability aspect. Overall, its a pretty interesting and bleak game, best to go in and play blind.

Look, I don't have a lot of time to tell you this, but this game is important. Historically speaking, it's doing more to push the boundaries of this medium than any other games in recent memory. Scalpel. Good. Now put your hands here and apply pressure.

We need this thing alive. It barely arrived to us at all, and its condition was not good. It's getting better, but it's going to die if we don't do something. I confess, I meant to tend to it sooner but I was distracted. I broke my promise. Take this flask and pour a few milliliters while I tilt its head back. Okay.

Listen, dammit. This game is a miracle. It cost more than a few fingernails to ferry this soul across the Styx, but it's here. The soul is still here. The heart is still beating. But every compromise made between this game as a piece of art and a commercial product will have been a waste if you don't turn those bills into bandages and staunch the flow.

And for God's sake, mind your bedside manner. Listen to its story, even if it's hard. Offer a kind word. Bring visitors. Hell, throw a party and invite anyone you know. We can argue another day whether the life of every game is worth saving -- whether every single idea deserves to see the light of day. Right now, I'm telling you that this one is in danger of bleeding out on the floor, and you can help.

A word of advice. You will want to do everything, save everyone, including yourself, all without compromising your morals. I don't know if it's possible. Likely, you will have to compromise one or all of those things just to see the end. That's okay. That's the way the game is meant to be played. You aren't missing things by failing. Don't be discouraged. Keep trying. Put any paltry comparisons to other difficult games out of your mind. You do not win by restarting again and again until you get it right. You will stumble through this performance, start to finish. Accidents happen. Actors die, understudies die, each is replaceable. But you have to see it through.

The show must go on. Keep the scalpel. When Mark calls your name, just walk onto the stage.

Playing Pathologic 2 is a truly unusual experience, a wholly enjoyable experience that is defined by gameplay that is made to be completely and utterly not enjoyable. The gameplay loop revolved around trying to balance out your commitment to curing the plague, fulfilling various obligations to the people in the town you're committed to saving as the doctor, and trying to manage your own survival on top of all of it. It creates a better system of morality without throwing out any sort of good/bad meter at you. You will have to decide for yourself whether you did the right thing in any situation and you WILL feel bad about some of your decisions.

Ask yourself, are you willing to do ANYTHING to survive? ANYTHING to do what's right? Is it worth dooming someone to death because you think your resources are better off treating someone else for their infection? Or perhaps to boost your own immunity to the plague so you can live to cure for just one more day? Do you cut open the body of a woman who died in the streets so you can attempt to brew a panacea from her tainted organs? Do you rob homes blind so you can afford bread for the day?

Does it even matter? Can one man truly change the course of nature? Or is nature simply doing what nature does and putting humanity in the crossfire? Do humans have a right to exist and abuse the Earth, or are we simply grass to be consumed by the almighty bull?

Anyway, Pathologic 2 is good. It's not fun, but that's not the point, it's engaging in ways that games truly are. It's immersive in a way that games almost never are, and the writing is confusing in a way that that makes it even more so. The world is going to hell, people talk in cryptic paragraphs, and you're just trying to make heads or tails of what's going on to at least try and save some of the people around you. it's beautiful, I love it, and maybe you will too.

I remember talking to a friend about how much I wish there was a game set during a plague. I said I wanted it to be super depressing with people dying all over the place and killing each other. I wanted to walk the sick streets and just feel the death around me (no I don't need help, go away), make hard choices, try my best to keep it together, and still feel everything spiraling out of control. And she was like ''hey I think you're talking about Pathologic''. Still took me quite a long time to give the game a try because I'm just so skeptical about video games lately and what's worth the money and what not, and I regret waiting so long and ever doubting this one.

This game is great. Loved every moment of it. It is everything my cold dead heart wished for, and now I want - no, NEED more. Pathologic deserves way more attention and support than it got. It's honestly a piece of art and quite easily one of the best games I have played.

My name is Artemy Burakh. I'm twenty-odd years old. My Lair is in the southeast section of the Town, where all the warehouses are, and I am not married. I work as a menkhu of the Kin, and I get home every day by 6 AM at the latest. I don't smoke, but I occasionally drink twyrine. I'm in bed by the time I max out my exhaustion meter, and make sure I get two hours of sleep, no matter what. After having an ampoule of warm morphine and doing about twenty minutes of tincture brewing before going to bed, I usually have no problems sleeping unless interrupted by a prophetic dream. Just like a baby, I wake up hungry and anxious in the morning. I was told not to get infected unless I fancy getting burned alive at my last check-up. I'm trying to explain that I'm a person who wishes to live a very long life. I take care not to trouble myself with any enemies, like winning and losing, that would make me waste my bandages and mend my clothes. That is how I deal with the townsfolk, and I know that is what keeps me out of the Theatre. Although, if I were to fight, me and my shotgun wouldn't lose to anyone.

After completing the game twice with varying playstyles and choices, I can safely say that this is one of the most fascinating games to be released in the last ten years.

Everything from the sheer commitment to its insane lore and plot to its devotion to making the player's suffering truly mean something shines as an example of how one can really toy with the conventions of this medium in a way that finds a perfect balance. It seldom feels conceited, and almost always feels deliberate in every step it makes.

It does lack quite a bit of polish (had to really go out of my way to avoid a constant crashing glitch near the end) but that's to be expected of a small team. If anything, it's more impressive that such a small team was able to make a game as fleshed-out and in-depth as this one.

While not for the faint of heart, Pathologic 2 is an absolutely worthwhile experience that will test your patience, grind you to the bone, and spit on you, and yet you'll keep coming back for more.

Playing this during Covid 19 makes it even more creepy.
In all seriousness, one of the best games I've played in a while. Really creepy atmosphere, and the soundtrack alone is fantastic. 10/10

I'll rewrite this when I actually finish it but god...no way for me to explain what i like about this game so much without sounding pretentious as hell so im just tossing that concern to the wind for now.

Listen. If you have the patience and resilience for this game and can appreciate good writing. Play this game. Just do it. I feel like I go into some kinda trance playing this, where everything is so hard but I'm just so curious about this world, about what's going to happen and what I need to do that I just can't stop until I'm thoroughly tired. It's more a heavy, atmospheric, philosophical experience than a game at this point and I fucking love that. The dialog is so well written with so many layers to it, sometimes I just pause and read it over and over again. It's gorgeous. I can hardly imagine how much more poetic it is in it's original language. I think this game is probably in my top 3 now and I'm not even finished.

Pathologic 2 is amazing, please play it.

One of the best games I've ever played. Is it difficult? I would say it's bloody hard. But suffering and missing things is an essential part of the game. A couple of times I've skipped some quests and let people die only because I wanted to survive myself. Heck, I was really close to abandon the main quest only because I wanted my character to live till the end of this. This game gives you difficult choices. It asks you provoking questions. It shows you a very cruel world, where you cannot be good, but you may try to be at least not bad. That's a brilliant example of a true RPG, where your choice matters.
Definitely recommend.

Playing through this while in home quarantine from covid-19 is one of the better gaming decisions I've made. It's as good and deep as all the other praise heaped on it, but what I wasn't expecting was the social and class commentary from it that is an excellent mirror of the issues much of the world is talking about now.

I still have to finish the game (that I only bought because of Ragnarox's review) but I'm really liking it and I want to support the developers.

I bought the game during Christmas' sales and what can I say? Now I have time to finish it since I'm italian and if I go out I might get the Plague.

10/10 Anxiety simulator, kind of like real life.

Fantastic game. So far I've killed 3 people, performed surgery, been attacked on the street, talked to kids, traded them marbles for a finger nail, gave a dog a leash, stolen bread from someones house, bought a bull for 200 dollars, talked to ghosts and poured blood onto the ground in order to grow herbs. I'm not even up to day 2 yet....

I would rate this game 9/10, but it may not be a 9 for everyone. Let me over-explain.

Pathologic 2 is a bit of a mash up of a survival game and plague doctor simulator. The game is both a remake and a sequel of the original Pathologic that was released in 2005. You don't need to play the original game as it references the original as having happened, but retells the story with big alterations and new plot points. The original game had three characters you could play with different views of the story, but this one only has the Haruspex at the moment. However, even just this one route will give you a full, complete game with an average of 30+ hours of gameplay. The developers are planning to eventually release the other two routes in the future, budget willing. This game was made with the idea that a game doesn't always have to be fun, sometimes a game can stretch you out and make you question what it's asking you to do. You will get stressed, you will get frustrated, and you will start to doubt whether you can even do everything the game is asking you to do, but that's the point. It will take well known tropes and rules of gaming and break them in an attempt to break you. It tries to make you feel as if you've actually gone through the experience of trying to save a town from a disease and how crushing, frustrating, and heartbreaking it can be to try to do that. This in turn makes the joy from the moments when things go right that much sweeter.

The town itself feels well thought out and it's populated with interesting characters with their own quirks. Some will be welcoming, some will be hostile, and some will seek to actively manipulate you. You'll develop your favorites and grow attached to them as you play through the game. The story itself, with 15+ years of time poured into writing and perfecting it, is incredibly engrossing with mystery layered on top of intrigue layered on top of borderline depressing Russian philosophy. There is combat in the game, but you're a doctor, not a trained fighter so you'll quickly get overwhelmed by more than one opponent. Instead, your goal is to survive at all costs, struggle to end the plague any way you can, and save as many people as you can along the way. Your own well-being comes second to your charges, you only need be alive, and you will have to pick which characters are important to you. The game has two economies, one involving money that slowly becomes unreliable and another that involves trading random items and trinkets with the townsfolk for more valuable items such as food or medicine. Almost every aspect of the game is designed to make you feel like you're just a doctor trying to save a town that's slowly falling apart. I will say that this can be a very difficult game if you try this on the intended difficulty, sometimes maddeningly so. However, the developers have added custom difficulties with sliders for almost everything they use to mess with you throughout the game. You may not get the full experience from the game by making it easier, but you can adjust these as you play if you feel certain aspects of the game are too difficult or too much of a cake walk.

There are some drawbacks that may prevent you from getting into the game, of course. The tutorial can be a bit confusing as to what is happening story-wise as it all takes the form of flashbacks and fever dreams as you ride a train back to your hometown. it does a good job of walking you through the mechanics of the game, but it's really confusing. I had to stop for a couple minutes at one point early on to question what I was looking at as it isn't clear what's going on and I still don't entirely get it. The language is rather theatrical and littered with metaphors, which could be a bit of a turn off for some people. You'll sometimes have to stop and reread what someone said a few times to understand the full meaning. However, the dialogue is greatly improved from the previous game. There's also one small graphical issue I noticed that bugged me, but not enough to pull me out of the game nor do I know if everyone gets it. I sometimes saw NPCs moving in the distance, but their walking animation wasn't loaded so they were just t-posing across the road. It didn't happen too often, but it happened enough for me to notice.

All in all, I'm probably just a fan boy of the game, but I'm willing to accept that since I believe this is a game that should be given a try. I can't really even put all of my thoughts about this game into this review so I'm going to cut it longish here. Try it on the intended difficulty, embrace your mistakes, and struggle your way to the end. This game is one that will stay with me, and I hope you, for a very long time.

Pathologic 2 is an intense, harrowing experience that won't leave your head for a while.
Sure, it's not made for everyone but those who like to see an interesting worldbuilding filled with uncompromised design choices will be glad to delve in this strange world. That said, it's also a really violent game with an haunting imagery, something you should know before playing it.
